CKD-EPI stands for Chronic Kidney Disease Epidemiology Collaboration and it was developed to improve the accuracy of eGFR calculation over the previous formulas like MDRD and Cockcroft-Gault, especially in the general population and in those with mild to moderate kidney dysfunction.

A CKD-EPI calculator is a tool that uses the creatinine and/or cystatin C levels in a person's blood to estimate their glomerular filtration rate (eGFR), which is a measure of kidney function. The calculator uses mathematical equations to determine the eGFR based on the person's creatinine and/or cystatin C level, age, gender, and race. The eGFR is then used to classify the person's kidney function into one of five stages, with stage 1 indicating normal kidney function and stage 5 indicating end-stage kidney disease.

Both creatinine and cystatin C are used to calculate the estimated glomerular filtration rate (eGFR), which is an indicator of kidney function. Creatinine, a waste product produced by muscle metabolism, is commonly used to measure kidney function, but its levels can be affected by muscle mass and age. Cystatin C, a protein produced by all nucleated cells, is not affected by muscle mass and has been shown to be a more accurate marker of kidney function, particularly in older adults or those with low muscle mass.

Therefore, using both creatinine and cystatin C in combination can provide a more accurate assessment of kidney function and help identify chronic kidney disease (CKD) earlier.
