CKD-EPI (2021) Cystatin C equation

Cystatin C is a protein that is produced by all nucleated cells in the body. It is a member of the cystatin family of proteins, which are known for their ability to inhibit cysteine proteases. The primary function of cystatin C is to control the activity of certain enzymes that are involved in breaking down proteins. Elevated levels of cystatin C in the blood have been associated with kidney disease, while decreased levels have been linked to certain neurological disorders. Additionally, cystatin C is also used as a marker for GFR (glomerular filtration rate) which is a measure of kidney function.

Cystatin C is used as part of the CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) equation to estimate glomerular filtration rate (GFR), which is a measure of kidney function. This equation is used to determine the stage of chronic kidney disease (CKD) a person is in, and to monitor changes in GFR over time.

One of the reasons cystatin C is used in the CKD-EPI equation is because it is a more accurate marker of kidney function than creatinine, which is the traditional marker used to estimate GFR. Cystatin C is not affected by muscle mass, diet, or other factors that can affect creatinine levels, making it a more reliable indicator of kidney function. Additionally, cystatin C is more stable in blood and can be measured more easily.

CKD-EPI equation uses cystatin C along with creatinine, age, sex, race, and other factors to estimate GFR, which is a better representation of kidney function than creatinine alone.

The accuracy of CKD-EPI calculations when using Cystatin C can be as high as 90%, compared with the Cockcroft-Gault Equation which has a typical accuracy of 85% or when using creatinine alone as a variable (~70% accuracy)
